PASSPORT & PASSPORT RELATED SERVICES
Application for a new Indian passport Eligibility: Indian citizens seeking services for Indian Passport, should ordinarily be a resident of Sumatra. Application for re-issue of passport can be submitted: - If the final validity of the passport is less than one year or within one year of expiry of the passport; - On exhaustion of pages or on change in appearance.
The Government of India has decided to introduce Machine Readable Passports (MRPs) w.e.f. 26th August, 2007. Passport applications will be processed at the Indian Consulate and data forwarded to the Central Processing Centre of the Ministry of External Affairs, New Delhi for printing of passports. Printed passports will be couriered back to the Consulate for scrutiny, signature and delivery to the applicants. This process may take three to four weeks. In order to avoid inconvenience, all applicants are advised to submit their applications well in advance.
Guidelines - Duly filled in Passport application form (for re-issue) (download) along with existing passport. Please use legible capital letters in blue ink. No column is to be left blank and the names should be in expanded form. - In case present passport is an additional booklet, previous passport is also required. - Attach self attested copy of Indonesian Residence Permit (KTP/KITAS/KIMAS/DINAS) with the application form. - Applicants are advised to submit two latest identical colour studio square photographs of 4.5 cm x 4.5 cm without border. Photographs should have light background (preferably white or off-white). Black and white photograph, photograph with coloured or dark glasses, dark background or in uniform, Polaroid prints or computer prints will not be accepted. The photo print should be clear, without any shadow and should have full face, front view and eyes open. The photograph should show the frontal face of the applicant and the face should occupy 70% of the photograph. There is a possibility of considerable delay in issue of a new passport should applicants fail to comply with the instructions relating to photograph - Applications for passport and miscellaneous services on passport are accepted ‘in person’ between 9.00 AM to 11.30 AM. on any working day. The applications can also be forwarded through registered post or reliable courier agency at the sole responsibility of the applicant. When an application is to be submitted through a person other than applicant, he should be duly authorized to do so. - In case of change in appearance, one copy of the photo should be attested by a Notary Public. - Photocopy of marriage certificate is to be submitted in case the name of their spouse (wife/husband) is to be included in the new passport. Please furnish Marriage certificate issued by the Registrar of Marriages or a notarized joint affidavit signed by both husband and -wife, along with a joint photograph and photocopy of the spouse (Husband/wife's) passport, if any. Marriage certificate issued by Church or Quasi's certificate (with a certified translation) in case of Muslims can also be submitted as proof of marriage. - The application should be supported by photocopies of relevant documents. Such photocopies should be self-attested and must not be sent by fax or e-mail. - Please note that additional booklet is no longer issued. Instead, a new passport valid for 10 years is being granted. - Your passport is a valuable document and is the property of the Government of India. It should always be kept either in your own custody or in the custody of a person duly authorized by you. If lost or destroyed, the facts and circumstances should be immediately reported to the nearest Indian Mission and the local police. Please keep a photocopy of your passport in a safe place. Without these details issuance of a duplicate passport may be delayed. - Your passport must not be altered or mutilated in anyway nor any endorsement made on it by any person other than a duly authorized official of the Government of India. - The Maximum life of an Indian international passport is 10 years. Please check the validity of your passport and ensure you obtain a fresh passport a few months before its expiry. In cases where the passport has already expired beyond a period of one year, applicant should submit an affidavit elaborating reasons for not submitting not applying for re-issue of passport within prescribed period.
Lost/Damaged Passport If you are applying for a new passport in lieu of a lost or damaged passport, please submit a letter explaining how the passport was lost or damaged along with the police report ( in case of lost passport), and six copies of duly filled in personal particulars proforma and seven passport size photographs. An affidavit will also required to be submitted (download) Applicants are advised to get in touch with the Consulate immediately and proceed as per advice.
ISSUE OF PASSPORT TO MINOR CHILDREN: EVERY CHILD SHOULD HAVE A SEPARATE PASSPORT. Children's names are no longer included in the parents' passports. A minor child is issued with a passport of maximum validity of five years from the date of issue or till 15 years of age, whichever is less. Following documents are required to be submitted:
POLICE CLEARANCE CERTIFICATE (PCC): In case the original passport is not issued by this Consulate, prior reference is required to be made to the original Passport Issuing Authority and the waiting period for issue of PCC could be up to 30 days. EMIGRATION CLEARANCE NOT REQUIRED (ECNR) STAMP : The requirement of ECNR has been done away with.
As change of address will require issue of a new passport, application for a new passport is required to be submitted along with requisite documents, including address proof (self-attested). Such a proof can be Election card / Current water bill / Electricity bill / Telephone bill / Bank statement / Income tax return stamped by IT Deptt ( all documents should show applicant's name). Fax copies/e-mail downloads of any supporting document is not acceptable. As per MEA’s recent instructions, any change in particulars on the laminated portion of the passport warrant issue of a new passport. Following additional documents will require to be submitted in case of change of names:

Please note that photocopies of all supporting documents should be self-attested. Such copies should be sent by post/courier along with application forms. Fax copies/e-mail downloads of supporting documents is not acceptable.
Applications for change in date of birth will not be entertained unless accompanied by a court order issued by a judicial officer not below the rank of first class magistrate in India.
INCOMPLETE/FALSE INFORMATION: Misrepresentation of facts may result in denial of passport/consular service. Applications received with incomplete details will remain unprocessed.
PAYMENT OF FEE: If the application is submitted in person, the fee can be paid in cash otherwise applicant can deposit the fee to Consulate’s bank account No. 008-128191-068 with HSBC, Medan and attach the deposit slip in original with the application.
NO REFUNDS : Fees, once received, cannot be refunded, even if the application is withdrawn or service cannot be rendered.
MODE OF SUBMISSION: Application may be submitted/collected either in person/authorized person or through a courier. In case application is sent by courier, make arrangement for the collection of passport from the Consulate. Applications sent through courier shall have a self-addressed envelope for return of the documents. Please note that this Consulate is not responsible for the safety of the documents entrusted to the courier.
